Days and hours of operation:

  • Wednesday, 6 – 9 p.m.

Eligibility guidelines:

  • Must have lived in Forsyth, Stokes, or Davie counties for at least the last three months
  • Cannot be eligible for state or federal sponsored healthcare insurance, including Veterans Administration, Medicaid, or Medicare
  • Generally cannot be eligible for healthcare insurance through your employer
  • To qualify as a new patient:
    All necessary information and financial guidelines must be met.  Please see the below brochures about the documentation that will be needed during the screening process
  • How to apply:
    Eligibility screenings are held every Tuesday and Wednesday afternoon, 1 – 4 p.m. at the Community Care Center. You do not need an appointment for the interview!

Walk-ins accepted?

Not for clinic visits, but we do accept walk-ins for STI testing

Fee?

No, donations are encouraged to support the Community Care Center, but no fee is required

Bilingual staff?

We have Spanish interpreters available each clinic night
