Services provided:

  • Direct assistance paying for medications to those with incomes up to 200% of the federal poverty level (monthly income of $2,127/individual and $2,873/couple in 2020). Medicare D plans pay first and we pay second. We provide participants with a card accepted at all pharmacies in Durham that covers medicines on our geriatric formulary, reducing the cost of approved medications to $2/generics and $5-$15/brand-name for a month’s supply.

     

  • Comprehensive medication therapy management (MTM) sessions with pharmacists trained in geriatrics. Most participants meet with us every six months to review each medication (prescription, over-the-counter and herbal) and to assess their medication knowledge, administration techniques, and adherence. We also discuss health promotion strategies, such as smoking cessation and sleep hygiene, and work closely with participants’ other providers to ensure that everyone is on the “same medication page.” This service is at no charge for seniors with incomes up to 300% of federal poverty level ($3,190/individual and $4,310/couple).

     

  • Tailored community referrals and care management to help older adults access other valuable programs, such as food assistance, medical transportation and cost-reducing Medicare programs.

  • Medicare insurance counseling to help maximize drug and/or health coverage and overcome obstacles to using benefits. We do this work in partnership with the NC Department of Insurance, as the Seniors’ Health Insurance Information Program (SHIIP) coordinating site for Durham County. We offer this service at no charge to anyone in Durham – regardless of age or income.

Mon, Wed, Th and Friday – 8:30 am – 5:00 pm; Tuesdays from 8:30 am – 8:00 pm

Eligibility guidelines:

  • Durham City or County residents 60 or older who met income guidelines above, or Medicare beneficiaries of any age or income.

Walk-ins: Yes, for basic screening, not full appointments

Fee: No, unless income is over 300% FPL and request MTM

Bilingual Staff: Yes, we currently use a language line
