Services provided:

• Vouchers to assist seniors with co-pays for medication of chronic health conditions
• Tracking of medication adherence for diabetics
• Donations of nutritional supplements, incontinence wear as available
• Monthly health topics mailed including info on Medicare Enrollment
• Satisfaction surveys
• Planning for new service of blood pressure checks (after COVID)
• Consultation with participating pharmacies for best use of voucher

Days and hours of operation:
Mon-Thurs 8:30 am-5 pm

Eligibility guidelines:

• Age 60+
• Chronic Health Condition
• No private insurance or Medicaid, Medicare ok
• 200% or below the Federal Poverty Level income,
• One time help for release from hospital
